Claim notification<\/h1>\n

Fill a notification of claim as soon as possible after the first doctor visit. When your claim has been registered, you will receive a confirmation email stating your claim number. The notification of claim shall be filled only once per injury.<\/p>\n

In case you need advice and support to fill in the form you may call PV Claims Administration customer service, tel. 06000 3990 (3, 42\u20ac\/call + lnc\/mcc). Our customer service number can be reached during November-March on weekdays from 8 am \u2013 8 pm and during April-October, including also weekends and public holidays, from 10 am \u2013 8 pm.<\/p>\n

The insurance covers necessary medical treatment of accidental injury within 2 years from date of injury.<\/p>\n
